Baptism under 7 years of age:
documents to gather
As part of the registration process, you will need to bring the
original copies of certain documents to the office. We will make a copy
of them then and there, and return them to you right away. For
baptism, you need to present the following:
•
Birth certificate of the child being baptized.
•
Signed and sealed affidavit of the first sponsor from that
individual’s own parish.
•
Signed and sealed affidavit of the second sponsor (unless you do
not have one) from that individual’s parish.
•
Confirmation certificate of the sponsor(s) from the parish where
the sacrament was received.
•
The certificate from having attended a baptismal talk within the
last year by the parents/legal guardians.
•
The certificate from having attended a baptismal talk within the
last year by the sponsor(s).
